Maximizing the Benefits of Postless Hip Arthroscopy: On-Off Traction Technique During Labral Repair to Restore the Labral Suction Seal.
Arthroscopy techniques Q1 PMID:38835468
Lucas J Ray, Mahad M Hassan, Ryan R Fader
英文摘要 Abstract
The acetabular labrum plays a key role in proper biomechanical hip function through creation or maintenance of a suction seal between the femoral head and acetabulum. The suction seal effect has been shown to provide stability within the hip, improve biomechanics, and decrease the chance for long-term development of osteoarthritis by optimizing function and force distribution within the hip. Femoral acetabular impingement syndrome damages the labrum and chondrolabral junction, thus negatively impacting the ability of the labrum to maintain native suction seal. Our technique describes the use of a postless hip arthroscopy table and the on-off traction technique throughout the labral repair, ensuring precise reduction of the labrum and restoration of the suction seal sequentially as anchors are placed.

Arthroscopic Repair of Acetabular Cartilage Delamination Using Chondral Nail Fixation in Patients With Femoroacetabular Impingement.
Arthroscopy techniques Q1 PMID:38835465
Hanmei Dong, Kang Tian, Guanying Gao et al.
英文摘要 Abstract
Acetabular cartilage delamination is commonly seen in patients with femoroacetabular impingement (FAI), especially ones with the cam deformity. However, the definition and classification of acetabular cartilage injuries caused by FAI to guide clinical treatment remain controversial. Moreover, treatment of acetabular cartilage damage always causes a dilemma for surgeon during surgery. We believe a reliable repair of the acetabular cartilage delamination will lead to a better long-term outcome for patients with FAI. In this Technical Note, we introduce the chondral nail fixation under hip arthroscopy for treating acetabular cartilage delamination in patients with FAI. This technique contributes to eliminating intra-articular unstable factors, preserving native cartilage as much as possible, and restoring cartilage surface intact at best.

英文摘要 Abstract
Hip arthroscopy continues to increase in popularity and has an ever-expanding range of indications; however, the steep learning curve introduces significant risk of iatrogenic chondrolabral injury when accessing the joint and establishing arthroscopic portals. This article presents a technique for establishing the modified midanterior portal and is particularly useful when the available space is tight. We present "the air-lift" as a safe and simple adjunct to standard portal creation when performing hip arthroscopy in the supine position.

Editorial Commentary: Waiting at Least 1 Month After an Intra-articular Corticosteroid Injection for Performing a Knee, Shoulder, and Hip Arthroscopy Could Minimize the Risk for Postoperative Infection: Platelet-Rich Plasma Is an Alternative.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38431029
Nikolaos K Paschos
英文摘要 Abstract
An intra-articular corticosteroid is associated with a higher risk of joint infection. Identifying the necessary time interval from the injection to the arthroscopic procedure that does not impose an additional risk of infection is critical for the safety of our patients. Increasing evidence suggests that there is a high risk of infection at the first 4 weeks after the injection, but it seems that this risk declines to normal levels after that period. Interestingly, this time interval of 4 weeks is comparable between knee, shoulder, and hip injection and subsequent arthroscopy. A delay of an arthroscopic intervention at any joint for at least 4 weeks is recommended. Finally, platelet-rich plasma (PRP) is equally effective as corticosteroids in terms of pain management and anti-inflammatory response, with the advantage of promoting healing. PRP may also be associated with a lower risk of postoperative infection in patients undergoing arthroscopy, representing a biologic alternative.

Editorial Commentary: Endoscopic Repair of Abductor Tendon Tears Results in Variable Patient Reported Outcomes and Generally Good to Excellent Results.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38431028
Timothy J Jackson
英文摘要 Abstract
Abductor tendon pathology is fairly common, with up to a 25% incidence in patients having total hip arthroplasty and 30% having hip arthroscopy. A systematic review of endoscopic abductor tendon repair demonstrated that as few as 41% of patients with endoscopic repair of abductor tendon tears achieve a patient acceptable satisfactory state, but a major limitation of systematic reviews is extreme heterogeneity between included studies. Surgical techniques and skills differ, as do tear severity and confounding pathology such as labral tears. Another limitation is a focus on patient-reported outcome measures (PROMs). PROMs are important (we prefer happy patients with poor healing to unhappy patients with healed repairs), but PROMs are also "subjective," and different cohorts of patients in different studies from different locations may have different perceptions or goals with regard to pain and function. As surgeons, we are able to observe gait, strength, and, with advanced imaging when indicated or for research purposes, healing. These, combined with PROMs, influence overall assessment of outcome. Experience and review of the literature show that endoscopic surgical repair of abductor tendon tears generally shows good or excellent results. If a patient has significant pain improvement and objectively improved gait, a calculation of an outcome threshold based on a subjective survey may not tell the full story.

Editorial Commentary: Clinical Equipoise Regarding Arthroscopic Management of Femoroacetabular Impingement With Tönnis Grade 2 Osteoarthritis May Mean We Don't Know or Don't Agree: It Does Not Mean We Don't Care.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38428697
John R Miller
英文摘要 Abstract
Management of patients with femoroacetabular impingement and moderate (Tönnis grade 2) osteoarthritis remains a debated topic. Outcomes show that such patients can benefit from hip arthroscopy, yet the improvement may not be as favorable as desired. Although certain factors, such as Tönnis grade 3 hip OA, older age, higher body mass index, bipolar cartilage defects, and joint space less than 2 mm, may influence surgeons to avoid arthroscopic treatment, the threshold for grade 2 OA is not as clear. Moreover, although radiographs may appear similar in patients with Tönnis grade 2, there may be a wide range of chondral damage seen arthroscopically. Thus, until higher-level studies are performed, it is the responsibility of the surgeon to delineate appropriate treatment strategies for individual patients in this category, and most of all, it is crucial for surgeons to set reasonable expectations for patients when considering hip arthroscopy. This highlights the importance of careful preoperative evaluation and patient education.

Editorial Commentary: Older Patients Without Degenerative Joint Disease Can Have Excellent Outcomes After Hip Arthroscopy.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38365124
John P Scanaliato, Andrew B Wolff
英文摘要 Abstract
Early research on hip arthroscopy for femoroacetabular impingement syndrome identified patient age as a risk factor for poor outcomes and heightened revision rates. However, more recent research challenges this dogma. Disparities between chronologic and physiologic age exist among patients. In addition, orthopaedic pathology (such as degenerative chondral or labral pathology) can correlate with age, so the use of age, and using age as a predictor of outcome, may be biased by this confounding effect. Older patients without degenerative joint disease can have excellent outcomes after hip arthroscopy. Recent research shows that patients older than the age of 40 years undergoing hip arthroscopy for femoroacetabular impingement syndrome demonstrate outcomes comparable with younger patients at 10-year follow-up. However, older patients were at greater risk for conversion to total hip arthroplasty. In our experience, in the absence of joint degeneration, patients older than the age of 40 years experience predictable and durable relief following arthroscopic management of femoroacetabular impingement syndrome. These outcomes result from sound surgical indications, appropriate preoperative counseling, and meticulous surgical technique. We ought not replace joints we can preserve, and age is not the most determinative factor regarding outcome of otherwise well-indicated and performed hip arthroscopy.

英文摘要 Abstract
Femoroacetabular impingement syndrome (FAIS) is a possible cause of early osteoarthritis, and restoration of normal anatomy can potentially prevent future major cartilage damage. Symptomatic cam lesions can lead to debonding of articular cartilage, resulting in superolateral cartilage lesions in 93% of patients. Patients with pincer lesions often exhibit cartilage lesions in the anterior and superolateral glenoid rim. Whereas the efficacy of prophylactic surgery remains uncertain, surgical intervention appears to yield superior short-term clinical outcomes compared with conservative treatment. Yet, there is a relatively high prevalence of asymptomatic individuals in the general population with either cam (25%) or pincer (67%) deformities, so prophylactic treatment of asymptomatic patients cannot be recommended. Symptomatic FAIS patients with Tönnis grades 0 and 1 (minimal hip arthritis) benefit from hip arthroscopy. Patients with higher grades of osteoarthritis may be unsuitable hip arthroscopy candidates and face a higher risk of conversion to hip replacement. The existing evidence regarding the treatment of patients with FAIS and Tönnis grade 2 or higher remains inconclusive. Generally, surgery tends to have favorable outcomes for younger patients with a normal body mass index, whereas nonsurgical alternatives should be explored in cases with severe joint space narrowing, Tönnis grade 3, and bilateral cartilage lesions.

Editorial Commentary: Primary Treatment of Hip Labral, Osseous, Chondral, Capsular, and Extra-Articular Pathology Is Essential: Outcomes Following Revision Are Less Predictable.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38342282

英文摘要 Abstract
Appropriate labral management is one of many procedures during hip arthroscopy that affects postoperative outcomes and revision rates. Both primary labral repair and reconstruction have been shown to have superior clinical and functional outcomes compared with labral debridement when treating unstable labral tears. Arthroscopic labral reconstruction is one of the most powerful techniques in the arsenal of complex hip-preservation surgeons, and although often reserved for the revision setting, when the native labrum is irreparable, a primary reconstruction may be indicated when the only alternatives are selective labral debridement or a suboptimal repair. Labral reconstruction, either in primary or revision procedures, is indicated when the existing labrum is deemed irreparable based on an intraoperative evaluation. Current indications for primary labral reconstruction, either in the primary or revision setting, include a calcified labrum, an irreparable mixed Seldes type 1 and 2 tear, or a hypoplastic labrum with less than 3 mm of viable tissue. Primary hip arthroscopy has been shown to have superior outcomes compared with revision hip arthroscopy, whether with labral repair or reconstruction. Finally, appropriate labral management is necessary but not always sufficient. Hip arthroscopy requires management of osseous deformities, with care taken to avoid under- and over-resection during both femoroplasty and acetabuloplasty; management of chondral injury; and management of the hip capsule with repair or plication. Consideration also must be given to potential extra-articular pain generators, such as abductor insufficiency, ischiofemoral impingement, lumbar spine disease, as well as deformities requiring open surgical correction such as acetabular dysplasia or pathologic femoral version. The primary goal is getting it right the first time.

英文摘要 Abstract
Femoroacetabular impingement syndrome can cause pain and difficulty with sexual activity. Arthroscopic treatment of femoroacetabular impingement can improve a patient's sexual health, as seen by improvement in patient-reported outcomes. This correlates with improved quality-of-life scores. Sexual function scores improve for both male and female patients after surgery, but improvement in female patients is greater, most likely because sexual positioning in female patients may involve more flexion, abduction, and rotation than male patients. Reviewing patient-reported outcomes can help provide context for the treating surgeon for discussion both preoperatively and postoperatively.

英文摘要 Abstract
PURPOSE: To assess the diagnostic capability of radiographs (XRs) to detect pincer lesions compared with 3-dimensional (3D) computed tomography scans in patients undergoing hip arthroscopy for femoroacetabular impingement syndrome (FAIS). METHODS: We performed a retrospective review of all patients who underwent hip arthroscopy for FAIS between September 1, 2020, and October 2, 2022. Preoperative imaging was reviewed. Pincer lesions were defined as a lateral center-edge angle greater than 40°; a Tönnis angle greater than 0°; the presence of the ischial spine, crossover, or posterior wall sign; and the presence of overcoverage greater than 80%. Under "select criteria," patients were classified as having a pincer lesion on XRs and 3D computed tomography reconstructions (CTRs) based on the lateral center-edge angle or Tönnis angle alone, whereas "all criteria" added the presence of the crossover sign and coverage percentage. Statistical analysis was performed to determine the diagnostic accuracy of XRs compared with 3D CTRs. RESULTS: A total of 69 patients met the inclusion criteria. There were 21 male patients (30.4%) and 48 female patients (69.6%). The mean age was 33 ± 13.5 years. χ CONCLUSIONS: In this study, we found that 3D CTR detected pincer lesions in patients undergoing hip arthroscopy for FAIS with significantly higher sensitivity than XRs alone. LEVEL OF EVIDENCE: Level III, retrospective cohort study.

Editorial Commentary: Irreparable, Symptomatic, Hip Labral Tears Should Be Reconstructed in Patients without Osteoarthritis. However, Comparison of the Two, as Well as Allograft versus Autograft Reconstruction, Is Challenging.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38296454
Justin R Zhu, Andrew E Jimenez
英文摘要 Abstract
Hip acetabular labrum plays a chondroprotective role by maintaining stability, providing a suction seal, and contributing to load distribution. In symptomatic patients without osteoarthritis, arthroscopic labral repair has strong clinical and biomechanical data to support its use. For irreparable tears, the labrum should be reconstructed. Biomechanically, labral reconstruction does not demonstrate superiority when compared to labral repair or the intact native labrum state. However, it is superior to labral excision. One challenge in making these comparisons is the heterogeneity in surgical technique. Labral reconstruction techniques can vary significantly. In terms of reconstruction, allograft offers the advantage of no donor site morbidity, but availability, costs, and risks must be considered. Autograft use is limited by donor site morbidity and increased operative time. Direct comparisons again show no clear evidence to suggest superiority of one over the other.

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38296453
Dan Cohen, Jansen Johnson, Olufemi R Ayeni
英文摘要 Abstract
Articular cartilage lesions of the acetabulum may result in significant pain and dysfunction for patients with treatment options consisting of either microfracture or various forms of cartilage restoration procedures. A systematic review of 529 patients demonstrated similar, if not lower, reoperation rates and patient-reported outcomes in patients receiving cartilage restoration procedures compared with microfracture. The primary outcomes examined in this review were reoperation rates and patient-reported outcomes with no clear mention of radiographic outcomes and no clearly defined indications as to who would benefit from a cartilage-based procedure. This raises the question as to whether there should be a consensus-based and standardized criteria established and standardized among the hip preservation expert community to evaluate the success of these cartilage restoration procedures from an imaging standpoint. These criteria can also be incorporated into a composite evaluation that combines clinical, imaging, and patient-reported outcomes to determine optimal patient candidacy for cartilage procedures as well. This would be a very useful steppingstone for much-needed future Level I randomized studies or prospective, registry-based data on this topic.

英文摘要 Abstract
Surgeons performing hip arthroscopy (HA) continue to look for solutions to address immediate postoperative pain and achieve a safe and efficient same-day discharge. Multiple studies have looked at modalities to achieve this, and we have learned that some methods are successful whereas others have a high-risk profile and should be avoided. The pericapsular nerve group (PENG) block is at the forefront of the literature. The PENG block is an easy-to-perform pericapsular nerve block that has been shown in prospective and retrospective studies to decrease acute postoperative pain. With modern ultrasound techniques, the PENG block has a short learning curve and acceptable patient safety profile. Other regional blocks have been extensively studied and are not without their drawbacks. Neuraxial anesthesia has shown decreased pain after HA but carries the risk of neuraxial complications and ambulatory discharge delay. Quadratus lumborum, lumbar plexus, and femoral nerve blocks can impact muscle strength and carry the risk of falls. Fascia iliaca blocks have shown the potential for increased pain and have been abandoned. The PENG block has been shown to be safe, but there are risks of inadvertent femoral nerve block if the probe or needle is out of plane. The PENG block does not seem to be the only answer to controlling post-HA pain but is one more weapon in our armamentarium to treat acute postoperative pain. Our current multimodal HA protocol consists of intravenous ketorolac, a PENG block, general endotracheal anesthesia, 4 mg of dexamethasone, 25 mg of ketamine at induction and 25 mg of ketamine slowly infused throughout the case, and post-free HA. We have a 100% same-day discharge rate, with no reported block complications and no groin complications, and for us, this method has proved to decrease the time to discharge.

Editorial Commentary: Capsule Repair Is Necessary at the Conclusion of Arthroscopy to Optimize Return to Full Activity.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38296441

英文摘要 Abstract
Hip capsulotomy and capsule repair have minimal effect on joint kinematics during activities of daily living. With low demands placed on the hip, the hip capsule may provide little to no role on hip stability or microinstability. But the majority, if not all, patients undergoing hip arthroscopy are not content with only resuming basic activities of daily living. Seeking to optimize the hip joint to its maximum capacity should be the goal. Capsule repair is necessary in most to achieve that goal.

Editorial Commentary: Concomitant Hip Arthroscopy and Periacetabular Osteotomy Treat Both the Cause and the Effects of Hip Dysplasia: The Best of Both Worlds.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38296440

英文摘要 Abstract
In the management of true hip dysplasia, the bony deformity is nearly always accompanied by, and often causal of, intra-articular pathology. It is strongly recommended to conduct a comprehensive preoperative evaluation as well as arthroscopic evaluation and treatment of coexisting hip joint conditions prior to performing an open Bernese periacetabular osteotomy. The osteotomy improves coverage of the femoral head and corrects the abnormal joint mechanics and loading patterns, which result in cartilage damage and subsequent osteoarthritis. Arthroscopy allows treatment of the intra-articular pathology. Combining the hip arthroscopy and periacetabular osteotomy procedures into a single surgical intervention, performed by either a single surgeon or a collaborative surgical team, allows us to address both the cause and the effects of dysplasia. This once-controversial combined technique has become a standard practice in the field with widespread adoption.

Editorial Commentary: Small Labral Size on Magnetic Resonance Imaging Is an Indication for Labral Reconstruction in the Setting of Revision Hip Arthroscopy.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38296438
Parth Lodhia, Mark Owen McConkey
英文摘要 Abstract
Revision hip arthroscopy is an increasingly common procedure as rates of primary hip arthroscopy rise. Etiologies for symptom recurrence may include residual femoroacetabular impingement, cam over-resection, labral pathology, chondral wear, adhesions, and instability. This has spawned sophisticated surgical techniques in hip arthroscopy including labral reconstruction. Indications for labral reconstruction in the revision setting obviously include labral deficiency. In addition, in the absence of other obvious reasons for failure of the primary procedure, a labral reconstruction should be considered for diminutive labra. The ability to predict labral size before surgery based on magnetic resonance imaging can be invaluable.
