Arthroscopic Treatment of Cam-Type Impingement for Femoroacetabular Impingement Using Patient's Own 1:1 Three-Dimensional Printed Hip Model Without the Use of Fluoroscopy.
Arthroscopy techniques Q1 PMID:38690335
Ryland Murphy, Ivan Wong
英文摘要 Abstract
The arthroscopic treatment of femoroacetabular impingement (FAI) has increased greatly in popularity over the past decades. Treatment involves the resection of abnormal bony morphology of the femoral head/neck (cam-type) and the acetabulum (pincer-type), which otherwise create damage from the pathologic contact between the 2 structures. More recently, in evaluating the postoperative success of FAI surgery, unsuccessful resection of the cam impingement has been identified as a leading cause for revision. To evaluate adequate cam resection intraoperatively, C-arm fluoroscopy is most commonly used. However, fluoroscopy has disadvantages, including its limited availability in smaller surgical centers, radiation exposure, and it only provides 2-dimensional information of a 3-dimensional problem. With the recent implementation of ultrasound-guided portal placement, a technique for adequate cam resection is the last barrier to eliminating the need for intraoperative imaging for FAI. We present a technique that uses a 1:1 3-dimensional printed model made from computed tomography scans that have the patient's unique anatomy, to better identify and quantify the resection of cam-type impingements. This technique is reproducible and can lead to better understanding of the cam resection for each individual patient. Further, when combined with ultrasound-guided portal placement, it eliminates the need for intraoperative fluoroscopy.

Capsular Management in Hip Arthroscopy: Interportal and T-Capsulotomy, Suspension, and Closure.
Arthroscopy techniques Q1 PMID:38584622
Dustin R Lee, Karissa N Simon, Bruce A Levy et al.
英文摘要 Abstract
Capsular management in hip arthroscopy has recently become a popular topic in the literature. Various approaches have been developed around the critical balance between safe and satisfactory exposure while maintaining hip joint stability and the restoration of capsular integrity at the conclusion of the case. Advocates for capsular closure recognize the role of the capsule in providing hip joint stability and aim to reestablish normal hip biomechanics through capsule preservation. Several recent studies have also shown capsular management strategies to influence both clinical outcomes and risk of revision surgery. We present an effective method for capsular management in hip arthroscopy that consistently allows excellent exposure and working space while allowing for facile, anatomic closure.

英文摘要 Abstract
Hip arthroscopy continues to increase in popularity and has an ever-expanding range of indications; however, the steep learning curve introduces significant risk of iatrogenic chondrolabral injury when accessing the joint and establishing arthroscopic portals. This article presents a technique for establishing the modified midanterior portal and is particularly useful when the available space is tight. We present "the air-lift" as a safe and simple adjunct to standard portal creation when performing hip arthroscopy in the supine position.

Editorial Commentary: Extreme Hip Labral Size (Both Small and Large) Shows Inferior Outcomes After Arthroscopic Labral Repair With Femoroacetabular Impingement Treatment.

英文摘要 Abstract
Arthroscopic management of hip femoroacetabular impingement shows reliable improvement in pain and patient-reported outcomes, high survivorship, and low conversion to total hip arthroplasty, particularly in the absence of dysplasia. Concomitant labral repair-or augmentation versus reconstruction when indicated- consistently shows better results than labral debridement. The effect of labral size on outcomes has been studies in several series. Some studies show that smaller labra show inferior outcomes after repair, others show labral size makes no difference in outcomes, and one showed that patients large labra actually had worse outcomes. Perhaps labral size at either extreme does worse. Labral size is likely to remain one of many factors worth evaluating when prognosticating repair outcomes or indicating when advanced reconstructive options are warranted.

英文摘要 Abstract
Hip arthroscopy has become increasingly popular in recent years and continues to grow as techniques and understanding of hip arthroscopy evolve. Needle hip arthroscopy is emerging as a technique that can offer potential advantages compared with a traditional arthroscope. These benefits include a higher degree field of view, lower profile design for easier maneuverability, decreased arthroscopic fluid, and potentially decreased postoperative pain and swelling. We herein present and describe a technique of needle hip arthroscopy as a viable option in the treatment of hip pathology.

英文摘要 Abstract
The Technical Note aims to present an arthroscopic capsular closure technique at the end of the hip arthroscopy. The technology employs a dual-channel approach and modified shoelace suture technique to continuously suture the hip capsule. Recent studies have indicated that routine intraoperative repair of the articular capsule at the end of the hip arthroscopy is advocated. However, the majority of the hip capsular closure techniques are relatively complex, time-consuming, and bring many complications, which has hindered their widespread use in clinical practice. Herein, we provide an arthroscopic capsular closure technique using the modified shoelace continuous suture in combination with a dual cannula for correcting hip instability during hip arthroscopic surgery of femoroacetabular impingement.

Editorial Commentary: Long-Term Follow-Up After Endoscopic Gluteal Repair Plus Hip Arthroscopy Shows Durable Results Using Validated Patient-Reported Outcome Scores That Largely Exceed the Minimal Clinically Important Difference and Patient Acceptable Symptom State.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38278461

英文摘要 Abstract
Gluteus medius and minimus tendon pathology is a common cause of lateral hip pain. In patients who are dissatisfied with their hip condition following nonsurgical treatment, gluteal repair has demonstrated excellent short-, mid-, and, now recently, long-term subjective patient-reported and objective clinician-measured outcomes. In patients with peritrochanteric hip pain, the proportion of their overall hip pain may be influenced by the hip joint due to conditions like femoroacetabular impingement syndrome, acetabular dysplasia, labral tears, and arthritis. Thus, surgical decision-making must include consideration of also addressing the joint at the same time as the gluteal repair. This is sometimes challenging due to the high frequency of observing labral injuries and cam/pincer/dysplasia morphology in patients without symptoms due to the "radiographic abnormalities." Labral pathology is also more prevalent in older patients, who happen to also be those individuals with symptomatic gluteal tendon pain. Both open and endoscopic approaches to the gluteal tendons have advantages and disadvantages without significant outcomes differences in the short- or mid-term. Long-term clinical follow-up of patients treated with endoscopic gluteal repair with or without concomitant hip arthroscopy should be included in large national and international prospective registries using validated, reliable, and responsive patient-reported outcome scores, with clinical importance assessed using the minimal clinically important difference, patient acceptable symptom state, substantial clinical benefit, and maximal outcome improvement.

Editorial Commentary: Capsular Management During Hip Arthroscopy Is Recommended, But a Limited Capsulotomy in Male Patients May Heal Without Closure.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38278460

英文摘要 Abstract
The hip capsule consists of the iliofemoral, ischiofemoral, and pubfemoral ligaments. The iliofemoral ligament is an important part of the anterior hip capsule that functions to stabilize the joint but is commonly incised in order to obtain access during hip arthroscopy, as described in techniques such as interportal, T, puncture, and periportal capsulotomy. For the most commonly used interportal capsulotomy, recent literature has advocated for closure of the capsule at the end of the surgery to avoid iatrogenic instability or microinstability. Systematic reviews and cohort studies have reported significantly better patient-reported outcomes after hip arthroscopy for femoroacetabular impingement syndrome with capsule closure compared to without capsule closure. However, recent high-level evidence from a randomized controlled trial demonstrated that in a predominantly male cohort there was no difference in patient-reported outcomes improvements or complications between patients undergoing hip arthroscopy for femoroacetabular impingement syndrome through an interportal capsulotomy who were randomized to receive capsule closure versus no capsule closure. Of note, male patients have inherently tighter and more stable joints than female patients and therefore are at lower risk for postoperative instability or microinstability from an interportal capsulotomy that does not properly heal. Also, if the capsule is not violated below the 3-o'clock position (for a right hip, or 9 o'clock for a left hip), there may be less risk to destabilizing the hip joint. A limited interportal capsulotomy in male patients could lead to healing in an unrepaired state.

Editorial Commentary: Magnetic Resonance Imaging May Underestimate Hip Femoral Version Versus Computed Tomography Scan: Both May Be Optimized Using 3-Dimensional Imaging.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38253294
Jay Moran, Andrew E Jimenez
英文摘要 Abstract
Hip arthroscopy has become increasingly popular in the treatment of femoroacetabular impingement syndrome. Careful preoperative planning including evaluation of acetabular and femoral version is necessary to optimize outcomes. Increased femoral anteversion has been associated with microinstability, and conversely, decreased femoral version may predispose to impingement. Computed tomography (CT) is considered the gold standard for femoral version measurements and preoperative planning. However, with recent advancements in technology, magnetic resonance imaging (MRI) has shown comparable measurements to CT imaging. In terms of advantages and disadvantages, CT requires radiation, albeit "low dose." MRI has a longer acquisition time, and movement of the patient may affect image quality and subsequent accuracy. MRI generally underestimates true version, probably as a result of patient positioning. Three-dimensional imaging could resolve this issue and may become the gold standard for both CT and MRI.

Editorial Commentary: Diagnosis of Hip Acetabular Retroversion Requires 3-Dimensional Imaging.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38231143
Elizabeth J Scott
英文摘要 Abstract
Diagnosis of global hip acetabular retroversion requires 3-dimensional imaging. Using anteroposterior pelvis radiographs, the crossover sign has been associated with retroversion. Recent research reports that the ischial spine sign and posterior wall sign also should be considered, yet even taking into account all 3 signs, the false-positive rate for retroversion is reported as greater than 35%. In truth, an anteroposterior radiograph is not sufficient for determination of acetabular retroversion. Retroversion is a 3-dimensional condition, and magnetic resonance imaging or computed tomography-based measurements are required. Thus, before we can determine the efficacy of hip arthroscopy versus periacetabular osteotomy for patients with hip impingement secondary to acetabular retroversion, we need to precisely measure to confirm our diagnosis.

Editorial Commentary: Younger Patients With Hip Femoroacetabular Impingement Syndrome and a Superolateral Cam Lesion Location May Require Aggressive Treatment to Prevent Post-Arthroscopy Total Hip Replacement.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38219140

英文摘要 Abstract
Hip femoroacetabular impingement syndrome is observed in 47% to 74% of patients with hip pain. Femoroacetabular impingement syndrome may result in osteoarthritis. It is well known that cam lesion volume and hip alpha angle are critical parameters determining patient outcomes. Recent research shows that a superolateral cam lesion location increases risk of hip arthroplasty at 5 years, and that this is more common in younger patients. The clinical relevance is that we may need to more aggressively treat superolateral cam lesions in younger patients, pending additional research to determine whether location is destiny.

Editorial Commentary: Physical Therapy Alone May Not Successfully Treat Patients With Hip Pain and Large α Angles or Decreased Femoral Anteversion Combined With Femoroacetabular Impingement Syndrome.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38219139

英文摘要 Abstract
Understanding the relation between spinopelvic (lumbopelvic) tilt and femoracetabular impingement syndrome (FAIS) is complex, and determining the optimal patient parameters that lead to successful nonoperative management is vital. Physical therapy (PT) focusing on core and posterior chain strengthening is often successful. PT can change the posterior tilt of the pelvis by 5° to 10°, allowing increased range of motion (ROM) and decreased impingement of the hip. However, PT does not change cam anatomy. Thus, PT alone may not sufficiently increase ROM in patients with cam-type impingement and large α angles or limited femoral anteversion. Pelvic compensation may lead to successful nonoperative management of FAIS, but not in all patients. Large-cam, high-flexion athletes with chronic hip pain should try PT. Yet, while some patients with large cam lesions may improve without surgery if femoral version and/or pelvic tilt ROM can be increased, surgery should not be excessively delayed in patients with poor prognostic factors for nonoperative management.

英文摘要 Abstract
The ligamentum teres (LT) is known to play a role as a secondary stabilizer of the hip joint. LT tears can be associated with hip instability. In patients with borderline developmental dysplasia of the hip (BDDH), the correlation between LT tears and microinstability is even more pronounced because of the increased mechanical stress placed on the ligament. This relationship may lead certain surgeons to consider new indications for LT reconstructions. However, caution is warranted regarding the potential role of LT reconstruction in these patients, particularly since the primary deficiency in BDDH is bony undercoverage. Addressing this bony undercoverage should be a primary consideration that may be supplemented with other procedures, which may include addressing soft-tissue injuries around the hip such as LT tears. This is especially the case in those patients with persistent symptoms after management of labral tears or LT disruption.

英文摘要 Abstract
As the field of arthroscopic hip preservation surgery grows, large high-quality registries represent a foundational study design for establishing whether hip arthroscopy is effective for patients with femoroacetabular impingement syndrome (FAIS). Original research publications from experienced high-volume surgeons tell us "Can it work". A registry tells us "Does it work?". The ability of preservation to truly preserve the joint, delay the arthritis process, and reduce the risk of arthroplasty requires long-term follow-up. A geographic registry can follow this. The registry represents the "real world", a heterogeneous set of variables pertaining to the doctor, patient, intervention, and outcome. The vast array of factors that can be analyzed before, during, and after surgery makes machine learning an ideal technique for analysis of large quantities of data. A global hip preservation surgery registry is a desirable and achievable goal. In order to optimally predict outcome of hip arthroscopy, given the known large number of patient- and hip-specific factors that influence outcomes, a deep learning model with tens of thousands of subjects for this medium-scale task would be needed. Measures of clinical relevance need to include more than just MCID (minimal clinically important difference), which is the lowest bar minimal threshold. Patient expectations often far exceed MCID-requiring other metrics like SCB (substantial clinical benefit), PASS (patient acceptable symptom state), and MOI (maximal outcome improvement). Registries should include validated, reliable, and responsive patient-reported outcome scores (e.g., International Hip Outcome Tool [iHOT-12]) with measures of clinical relevance and expectations assessed routinely. The United Kingdom's NAHR (Non-Arthroplasty Hip Registry) and Denmark's DHAR (Danish Hip Arthroscopy Registry) are the two largest geography-based registries in current hip preservation research both with 11 years of patient enrollment.

Editorial Commentary: Arthroscopy With Labral Preservation or Reconstruction Plus Capsular Closure Shows Excellent Outcome in Patients With Borderline Hip Dysplasia and is Less Invasive Than Periacetabular Osteotomy.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38219131

英文摘要 Abstract
Borderline developmental dysplasia of the hip (BDDH), or borderline hip dysplasia (BHD), traditionally characterized by a relatively low lateral center-edge angle (LCEA), presents a complex challenge in treatment due to its multifaceted etiology involving instability, femoroacetabular impingement (FAI), or a combination thereof. The optimal approach to managing adult BDDH remains a subject of significant debate. Periacetabular osteotomy (PAO) is considered the preferred treatment for addressing severe acetabular under-coverage, as it rectifies the underlying bony deficiency and promotes stability. However, the treatment of BDDH is less straightforward, particularly with the advancement of hip arthroscopy techniques, specifically labral preservation and capsular management. Given the minimally invasive and rapid postoperative rehabilitation advantage for arthroscopic procedures over open surgeries, numerous attempts have been undertaken in this context. Research has revealed favorable patient-reported outcomes (PROs), low failure rates, and a significant proportion of returning to sports (RTS) after arthroscopic management for patients with BDDH. Although we might in the right direction, Level I evidence studies are needed to comprehensively compare long-term PROs, failure rates, and rates of RTS between arthroscopy and PAO in the treatment of BDDH.

At least 3 of 4 athletes will not only return to sport after hip arthroscopy but also maintain durability in their sport regardless of high-impact activity. Hoping our injured athletes can return to sport after their injury would prove an insufficient answer to our injured patients' expectations of a mathematically precise answer. As it turns out, Mathematical Hope exists as a term to describe the way we calculate probability. Because scientific research has shed light on the great outcomes that can be obtained from the arthroscopic treatment of a joint, we are now able to witness amazing comeback stories in the news and incredible outcomes from short- to long-term in scientific journals. Hip arthroscopy has now been supported by numerous research publications, turning our doubts into certainties, as high return to sport rates at high levels of performance are increasingly published.

英文摘要 Abstract
Which patients will benefit most from hip arthroscopy? Careful patient selection and conservative indications, such as patients with an alpha angle of 60° or greater or a lateral center-edge angle of 40° or greater who fail a trial of conservative treatment, may benefit from hip arthroscopy for femoroacetabular impingement (FAI). In female patients in particular, a lower body mass index (BMI) will predict the most benefit from arthroscopic treatment. That said, patients with a higher BMI can also substantially improve after treatment of FAI. The true art of medicine is determining indications for an individual patient in addition to providing evidence-based counseling and education. We must not forget that sometimes "any improvement" can be a good outcome for a patient who is in pain.

英文摘要 Abstract
The presence of hip paralabral cysts on magnetic resonance imaging is associated with labral tears and more extensive chondral damage. The 2-year follow-up results of patients with paralabral cysts are equivalent to patients who do not have paralabral cysts. However, since paralabral cysts are associated with more prearthritic hip cartilage damage, surgically addressing these hips sooner could yield better outcomes. Further research is needed to see if performing hip arthroscopy on patients with paralabral cysts more urgently yields better long-term outcomes than delaying surgery.

英文摘要 Abstract
From the lumbar spine to foot, the joints of the lower extremity are all intimately connected. Their movements are synchronized in a complex biomechanical dance. Pain in one joint tends to affect the joint above and joint below. Understanding the influence of adjacent-joint disease on the hip can help us better counsel patients undergoing hip surgery. Low-back pathology has been shown to negatively influence outcomes after hip arthroscopy. Ipsilateral knee pain appears to do the same. Patients with femoroacetabular impingement syndrome with ipsilateral knee pain should be counseled that their outcomes may not be quite as good as those without knee pain, but they should be able to expect meaningful improvement of their symptoms, both at the hip and at the knee after hip arthroscopy.

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38219101
Bradley M Kruckeberg, Marc J Philippon
英文摘要 Abstract
Hip labral reconstruction is indicated for hypoplastic, ossified, or irreparable labral tears in the primary and revision settings. Arthroscopic reconstruction for insufficient labral tissue requires advanced surgical techniques to restore hip biomechanics and re-establish the suction seal. With the growing number of arthroscopic hip procedures being performed, this is an increasingly familiar scenario. In our experience, the iliotibial band (ITB) autograft provides a safe and effective technique for labral reconstruction at 10-year clinical follow-up. Although the harvest requires an additional incision, the graft is incredibly versatile and can be harvested at any size to address the labral deficiency. Despite the concerns for donor-site morbidity, our extensive experience shows this is incredibly rare. In addition, concomitant pathology, such as greater trochanteric bursitis, can be addressed through this incision. Other grafts can be used for labral reconstruction, such as the indirect head of the rectus femoris tendon, but this is often limited to smaller labral defects less than 1 cm. ITB autograft shows excellent mid- to long-term outcomes, and second-look surgeries show excellent incorporation of the ITB autograft. And, in contrast to allograft, autograft tissue has demonstrated lower revision rates. The type of autograft used is per surgeon discretion based on experience and preference. In our hands, ITB is optimal due to proven effectiveness, durability, versatility, and limited donor-site morbidity.

英文摘要 Abstract
Current procedural terminology codes and assigned relative value units associated with arthroscopic hip surgery lag behind other joints in accurately describing, and often undervaluing, what surgery entails. Hip arthroscopy is expensive, and, to address inequity, procedural cost drivers require review. Consumable implants and operating room (OR) time drive the costs associated with the procedure. Hospitals, healthcare payors, patients, and surgeons all benefit from increasing OR efficiency and reducing equipment cost. However, the patient loses if financial strategy supersedes care delivery, and it is wrong to cut necessary use of consumables to save money. Fewer anchors is not the answer (yet we should use reusable, nonimplantable supplies when feasible). The greater opportunity to lower costs is improved OR efficiency, requiring a team approach with buy-in from perioperative, anesthesia, surgical staff, and administrators. OR time is a consistent driver of cost across every type of orthopaedic surgery. Studies evaluating strategies for OR efficiency in hip arthroscopy will benefit the field. By leading this effort, surgeons could be best positioned to address inadequate relative value units.

Editorial Commentary: In Terms of Hip Arthroscopy Long-term Outcomes, Intra-articular Cartilage Damage Is More Important Than Hip Labral Treatment Method: The Second Habit of Highly Effective Surgeons.

英文摘要 Abstract
There is a large debate revolving around the optimal surgical management of hip labral tears. Currently, the literature tends to gravitate toward labral repair as the mainstay of treatment, followed by labral reconstruction when repair is not feasible. Some studies assert that there is no difference between labral repair versus debridement, whereas others support repair. Other studies support labral reconstruction. In the end, it is the quality of the cartilage that may be most important. Intra-articular cartilage damage is more important than the labral treatment method when it comes to hip arthroscopy long-term outcomes.

Editorial Commentary: Femoral Version and Capsular Thickness Analysis in Hip Preservation Surgery-Surgical Indications Are as Important as Surgical Technique.
Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association Q1 PMID:38123275

英文摘要 Abstract
Optimal treatment of patients with femoroacetabular impingement syndrome requires both thoughtful decision-making and skillful operative technique. Traditional evidence-based literature and routine clinical practice overemphasize the role of the alpha angle on the femoral side and lateral center edge angle on the acetabular side. Femoral and acetabular version are critical values that influence outcomes and warrant measurement and consideration. Without version analysis, an uniformed, possibly poor, decision may be made. The literature and clinical practice also place dichotomous emphasis on labral (torn/intact) and capsular (torn/intact) integrity, with minimal appreciation of the morphological details of both. Not all capsules are created equal. "Normal" capsule thickness is a nebulous concept, with thinner anterior capsules more prone to anterior instability. Intuitively, it biomechanically stands to reason that excessive femoral anteversion (and excessive anterior cranial and central acetabular version) would place additional stress on the anterior capsule. Excessive femoral anteversion is associated with a thinner anterior capsule. Whether the latter is a reactive process (implies causation) or simply 2 concordant metrics (only correlation) has yet to be determined. In patients with nonarthritic hip pain, comprehensive quantitative consideration of both femoral and acetabular version and capsular thickness determines the optimal hip preservation procedure. Surgical indications are as important as surgical technique.
